Effective September 1, 2025, Texas House Bill 4224 added Section 181.105 to the Texas Health & Safety Code. Under this law, Thriving Child Center, as a covered entity, is required to prominently post detailed instructions on how consumers can:

  1. Request their health care records;

  2. Contact the licensing or disciplinary authority that oversees our services;

  3. File a consumer complaint.

Please review the information below so you understand your rights and available resources.

Requesting Your Health Care Records

You have the right to request access to your health care records maintained by Thriving Child Center.

To request your records:

  • Email: info@thrivingchildcenter.com

  • Please include: Your full name, date of birth, best contact email/phone, and the specific records you are requesting.

We will respond to your request in accordance with applicable Texas law and federal HIPAA regulations.

Contacting the Licensing & Oversight Authority

If you have questions about licensure or wish to contact the authority responsible for licensing and regulating our practice, you may reach:
